I understand and agree that this request is subject to appropriate verification and that this request may be refused if such verification fails in accordance with the California Consumer Privacy Act (CCPA).

I understand and agree WOW HD DE’s Privacy Policy and Terms of Use govern my request to the extent they are not in conflict with the CCPA.

I understand that WOW HD DE will endeavor to acknowledge receipt of my request within 10 days of my request, and comply within 45 days of my request. I further understand that if circumstances warrant, it may take up to 90 days for WOW HD DE to comply with my request.
